POSITION SUMMARY:
To provide quality care to
clients in their home.
The scope of practice includes direct client care and
advocacy.

Each employee is expected to maintain open, honest and
appropriate confidential communications with all customer

ESSENTIAL DUTIES AND RESPONSIB! ILITIES

Provides care in accordance with professional standards of
practice.

Maintains the level of clinical expertise necessary to meet
the needs of the home care population.

Maintains patient confidentiality as required by HIPAA.

Complies with infection control in accordance with Agency
policy.

Seeks out additional supervision or resources as needed.

Submits patient visit schedule timely and accurately to
ensure adequate patient care.

Will notify scheduler of availability and any updates to
their schedule as their day progresses.

Provides home health aid supervision in accordance with
Federal/State regulations and Agency policy as assigned.

Reports clinical status to primary clinician and/or other
appropriate staff.

Performs any other duty or responsibility assigned by
his/her supervisor.

Check company email on normally scheduled days at a min! imum
of the beginning and ending of their normal scheduled day
and responds ASAP but a maximum of within 24 hours

Provides effective case management of patients on caseload
with focus on meeting and/or exceeding agency quality

outcomes, included but not limited to:
o Case Mix Weight

o Re-Certification rate

o Target visits per episode

o Acute Care Re-Hospitalization

PATIENT CARE

Provides care in manner that assures above average patient
satisfaction outcomes as measured by HHCHPS and in
accordance with professional standards of practice.

Adheres to Agency established protocols for patient care (CHF
protocol, Complex Wound Care, etc.)

Completes clinical assessments per Federal/State
regulations, Agency policy, and professional best practices.

Develops an individual plan of care including patient
specific goals for each admission.

Follow! s the established plan of care.

Completes reassessment visit p! er Federal regulation and
Agency policy.

Establishes goals that are measurable, attainable and
pertinent to patient's condition and rehabilitation
potential.

Makes appropriate referrals for other services within the
agency and community.

Updates and revises the plan of care and interventions as
appropriate.

Provides appropriate teaching interventions to each client's
status as appropriate.

Develops discharge plans.

Demonstrates knowledge of contraindications/precautions in
relation to diagnosis and/or treatment.

Communicates with primary clinician and other clinical team
members as needed to promote optimal patient care.

Participates and documents case conferences with other
clinical team members as needed.

DOCUMENTATION

Completes documentation in accordance with Federal/State
regulations, Agency Policy and per professional best
practices.
Documents client's level of understanding and response to
teaching on every visit.

Documents functional status in terms that are measurable.

Documents on each visit client's progress towards goals
including barriers that impede progress.

Documents a skill for each billable visit.

Documents at point of care with the completion of the note
the same day as the visit.

Complies with agency policy regarding computer transfers.

Any other tasks assigned by supervisor.

QUALIFICATIONS

Experience, Education and Licensure

Graduate of School of Physical Therapy accredited by the
American Physical Therapy Association. Licensed to practice
Physical Therapy in the state where services are rendered.
One year of experience as a staff Physical Therapist under
qualified supervision in a hospital, clinic or certified
home care agency. Must be familiar with MS Word, Outlook and!
Excel.

Unless otherwise agreed upon by employer, employee! must
have a valid drivers license. Furthermore, employee shall
have use of a vehicle during working times and insurance as
a driver of that automobile.

Language Skills

Ability to read and interpret documents such as safety
rules, operating and maintenance instructions, and procedure
manuals. Ability to write routine reports and
correspondence.

Ability to speak effectively before groups of customers or
employees of organization.
