Qualifications
Job Summary
Referral requests are assessed for clinical appropriateness to determine acceptance for home health services and screened to ensure certified home health guidelines are met. Receive and document medical orders, accept for services, and fully process referrals. Review and reconcile physicians' orders and update medical data in the computerized system. Communicate pertinent information to involved parties according to Intake and agency guidelines. Primary contact with physicians for current and potential patients. Ensure that physician orders are received and communicated to appropriate staff. Maintain professional and collaborative external relations with referring physicians.
Key Responsibilities
Referral requests are assessed for clinical appropriateness to determine acceptance for services according to PHC standards.
Physicians' orders for home care are reconciled with initial r! eferral and reviewed for clinical significance as needed.
Referral requests for services are processed ensuring all information is complete and accurate according to Intake policies as needed.
Clinical and non-clinical data is updated and corrected in the computer system, as appropriate.
Referrals are screened to ensure certified home care guidelines are met.
All referrals with next day SOC, whether generated by liaison or other source, are completed and finalized by end of day.
All referrals with next day SOC, whether generated by liaison or other source, have physician orders by end of day or branch is notified that referral is incomplete.
Referrals from PHH Community Liaisons are reviewed, accepted and the transactions are completed according to department standards.
Incoming verbal physician orders are received and processed. New orders are communicated to relevant staff. Orders are entered into computer system and verbally communicated to ap! propriate staff.
