As a member of the Massachusetts Eye and Ear Infirmary Care Coordination team, the care coordination nurse (CCN) assumes major responsibilities for! coordinating and overseeing the discharge plans for patients requiring long term care facilities or complex home health nursing supports. Complies with all Federal, State and local regulations and laws governing the performance of this position within the department. Adheres to all Infirmary policies and procedures. Each care coordination nurse holds BSN degree from an accredited school of nursing and is licensed in Massachusetts.
RN with BS or BSN preferred
Experience: Minimum of four years of nursing experience with at least one year inpatient and two years of community health. Familiarity with Medicare regulations and third party reimbursement structures. Familiarity with health care needs of ophthalmology, otolaryngology and head and neck cancer patients. Previous experience with hospital-based care coordination programs
