Under the supervision of the Clinical Team Manager (CTM) and within the departmental and the organization's policies and procedures, assists the Manager in day-to-day operations of the Team; pro! vides administrative and secretarial support; may supervise volunteer(s); adhering to organization's confidentiality policy regarding patient and personnel information.

DUTIES

Answers and triages all telephone calls from CTMs, IDT, and patient/family members acting as a liaison to ensure timely response to patient-related issues.
Interacts with contracted agencies and with vendors to order durable medical equipment and disposable supplies for patients, nurses, and HHAs.
Maintains accurate and current patient information in clinical software
Creates and generates statistical reports for CTMs
Generates reports and tracks all overdue POC and verbal orders, making necessary telephone calls to remind physicians of need
Participates in IDT meetings; performs all report preparation for IDT meetings; sets up room as necessary,
Tracks and reminds RNCMs of recertifications that are due by clinician within 30 days.
Creates and updates all ! RNCM caseload sheets (patient lists) for RNCMs
Coordinates! , completes and faxes order requests for all C-lab home blood draws and documents confirmation number in chart
Documents all changes in patient status to appropriate sources, i.e. Hospice Pharmacia, Fiscal, vendors and MDs
Prints, collates, and distributes progress notes for all Facility team patients
Logs all necessary visits and tuck-in calls for DEN triage schedule
Compares existing triage log and answering service log alerting CTMs of any differences
Checks triage phone line for triage nurse's messages, erase at end of the day
Processes medication order renewals with Hospice Pharmacia via Xeris website
Reviews Hospice Pharmacia report 2X/day for physician signature needed, distributes to all teams as necessary and makes MD reminder telephone calls as necessary
Reviews and corrects Hospice Pharmacia Billable Days Report making all necessary date and level of care corrections
Performs routine typing of reports, correspondence, and distr! ibution of memos as needed
Creates new templates or tools for job processes as needed
Monitors, organizes, and maintains copies of medical forms and documents in cart in clinical area
Monitors and orders all necessary medical forms in bookcase in clinical area
Gathers information, types, and distributes weekly Days/Eves/Nights schedule and triage nurse schedule

QUALIFICATIONS

Maintains patient/family confidentiality.
Coordinates services and monitors effective use of Hospice Medicare/Medicaid benefit to insure cost effective delivery of care. Assists Clinical Manager to monitor financial expenditures regarding patient care.
Assists Clinical manager in ensuring compliance with regulation regarding documentation and care plan management.
Participates in clinical applicant interviews as requested.
Provides phone support to scheduled staff.
Supports communication of patient related information using appropriate technology: e! mail, text, phone, and voicemail
Participates in medical record aud! iting process to evaluate medical record documentation.
Verbally interprets plan of care to all care providers and assesses compliance.
Accurately documents in the patient medical record according to HopeHealth policy and procedure.
Establishes priorities to maintain continuity of Hospice patient care.
Assumes additional responsibilities as may be assigned by Clinical Team Manager

Responsibilities and Functions - Clinical LPN
Adheres to standards of nursing practice as set forth by the American Nurses Association and Massachusetts. Demonstrates an understanding of the hospice philosophy of care, as well as the policies and practices as set forth by HopeHealth.
Maintains competency level in the job, specific to nursing skills identified yearly by HopeHealth.
Under the supervision and/or direction of an RN, assesses patient and family status based on physiological, emotional, spiritual, and psychosocial needs presented; reports changes to ! RN and/or physician.
Maintains current physician's orders and care plans as per Hospice policy.
Instructs the caregiver in the care of the terminally ill based on individual personal differences, educational background, and previous experience.
If not already trained and competent in Peripheral IV insertion /maintenance, central line access/maintenance, will seek education and training for such skills within 3 months of employment; maintains competency in same; adheres to all related IV policies, procedures, and standards of practice.
Assumes additional responsibilities as may be assigned by Clinical Team Manager

EXPERIENCE REQUIRED

Computer literate in Windows environment required; keyboarding ability essential; working knowledge of office equipment essential
LPN licensed in Massachusetts.
Excellent assessment skills.
