Marathon Health, Inc. and Tufts University have partnered to offer an on-site health and wellness center for their employees and spouses at their Medford campus. This program is designed to he! lp inspire people to lead healthier lives.
We are seeking a full-time Registered Nurse who has knowledge and experience in health risk assessments, lifestyle risk reduction, disease management and health promotion.
Your role will be to work with the Tuft’s employees and spouses to achieve their personal health goals. You will be provided with state-of-art technology, protocols and information resources to support the development of effective, individualized health plans. The coaching approach employed is based on the principles of behavioral change and motivational interviewing.
RESPONSIBILITIES
The Health Coach is responsible for reaching out to build and maintain effective relationships with Tufts employees and spouses.
Provides individual health risk assessments.
Analyzes the individual’s health data to help the person prioritize and establish personal health goal(s).
Provides ongoing coaching, teaching, ! and social support to help individuals build self-efficacy ski! lls and achieve personal health goal(s).
Uses national clinical guidelines and standards to provide risk reduction and disease management services to individuals with chronic conditions and/or complex medical situations.
Effectively collaborates and integrates with healthcare professionals, other suppliers of corporate health services, and community resources.
QUALIFICATIONS
Must be licensed as a registered nurse with a minimum of five years professional, clinical experience.
Knowledge and experience in health risk assessments, lifestyle risk reduction, disease management and health promotion required.
This is a full-time position (M - F) located at the Medford Campus with some travel required. No on-call or weekends. Excellent compensation and full benefits offered.
