As part of the Lindemann Cambridge Somerville Site Office, incumbent will assess, plan, and evaluate potential clients for DMH services, such as Community Based Flexible Sup! ports, Case Management and Clubhouse. Participates in individual and multidisciplinary meetings to facilitate eligibility process for potential clients of Lindemann Cambridge Somerville Site. Conducts face-to-face interviews and thorough assessment including, but not limited to, psychiatric and medical history, educational and vocational background, family history, criminal background and any risks.
May be required to maintain a caseload of DMH eligible clients for both short and long term case management services.

1. Conducts face-to-face Needs and Means assessments, and gathers information on all SAU applications within specified time frame as established at the time of application.
2. Provides outreach and engagement to clients in the eligibility process as part of the Means and Needs evaluation in hospitals, treatment programs, ! and shelters.
3. Provides Short Term Case Management for I! ndividuals who have been determined eligible for DMH services but no service available by assisting with applying for Entitlements and establishes initial link with Medical and Behavioral Health providers when indicated. Meets with client as part of the eligibility process.
4. Completes all MHIS electronic entries and all required paperwork.
5. Meets weekly with supervisors to discuss status of current intakes and assignment of eligible cases.
6. Acts as a short-term case manager for eligible clients on the site waitlist until permanent assignments are made.
7. Participates in departmental staff meetings in addition to clinical meetings related to potential case management clients.
8. Ensures that all required documentation is completed in a thorough and comprehensive manner. Ensures that all required information in MHIS is completed and up to date.
9. Maintains continuing education as required by license or contractual obligation.
10. Performs a! ll other duties as assigned.

Qualifications:MINIMUM ENTRANCE REQUIREMENTS:
REQUIRED WORK EXPERIENCE: At least four years of full-time, or equivalent part-time, professional experience in human services work or social work. Based on assignment, at least one year of this work experience must have been in one of the following specialty areas. Refer to the individual position description for specialized work experience required.

  • Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: The year of specialized professional experience must have included the clinical care and treatment of psychiatric or emotionally disturbed individuals.
  • Department of Mental Retardation assignments only: The year of specialized professional experience must have involved working with individuals with mental retardation or other developmental disabilities.
SUBSTITUTIONS:
  • A Bachelor! 's or higher degree in social work, psychology, sociology, counseling, ! counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education may be substituted for two years of the required experience on the basis of two years of education for one year of experience.
  • Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: Degree substitutions must be in social work, psychology, sociology, counseling, counseling education, rehabilitation, rehabilitation counseling, human services, education of the physically or emotionally handicapped, education of the multiple handicapped, or education of the learning disabled.
  • Department of Mental Retardation assignments only: - A current and valid Massachusetts license or registration as a physical therapist, speech-language path! ologist, occupational therapist, audiologist, registered nurse, dietitian, or social worker may be substituted for two years of the required experience.
  • A current and valid Massachusetts license or registration as a physical therapy assistant or occupational therapy assistant may be substituted for one year of the required experience.
  • One year of education equals 30 semester hours. Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed.
  • No substitutions will be permitted for the required year of specialized professional experience.
REQUIRED EDUCATION AND LICENSES:
  • One of the following is required for Qualified Mental Retardation Professional assignments in the Department of Mental Retardation only:
  • A current and valid Massachusetts license or registration as a physical therapist, speech-language pathologist, occupational therapist, audiologist, regist! ered nurse, dietitian, or social worker, OR
  • A Bachelor's or hi! gher degree in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education.
  • Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: A Bachelor's or higher degree in social work, psychology, sociology, counseling, counseling education, rehabilitation, rehabilitation counseling, human services, education of the physically or emotionally handicapped, education of the multiple handicapped, or education of the learning disabled is required.
  • Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Class D Motor Vehicle Operator's license or the equivalen! t from another state.
Preferred Qualifications:
1. Demonstrates helping skills in the areas of advocacy, engagement, outreach, and supportive counseling skills to persons with severe and persistent mental illness.
2. Demonstrates knowledge of environmental and psychosocial problems of populations with severe and or persistent mental illness.
3. Demonstrates ability to recognize clinical symptoms and disorders associated individuals diagnosed with mental illness.
4. Demonstrates knowledge of federal state, and local human service agency that will provide needed in the areas of financial, medical, vocational, and housing benefits and services to the identified population.
5. Demonstrates flexibility in response to the changing needs of the identified population.
6. Demonstrates the ability to work with individuals of diverse cultures while respecting their values and ethos.
7. Demonstrates good verbal and written communication ski! lls.

Comments:
Full Time 40 hour position

! Hours: 8:30am to 5:00pm
Days off: Saturday and Sunday
