is seeking Home Health and/or Hospice Physical Therapist to join our team at a location near you! We currently have a Per Diem opportunity. Come join a community of caring health professionals!
Job Responsibilities
As a
Therapist
, you will provide clinical care to an assigned group of patients and families in the home setting. You will be responsible for ensuring along with an interdisciplinary team that patients and families are provided excellent care. Responsibilities include:
Communication with the physician to decide the initial plan of care based on patient assessment and physician's orders
Evaluation and treatment of patients using the most current technology and practices
Participation in clinical outcomes monitoring, and agency performance improvement initiatives
Experience working with a multi-disciplinary team and personal patient interaction

Qualifications
Job Requirements
Minimum of 1 year acute care experience required
Minimum of 1 year recent Home Health and/or Hospice experience required
Home Health RNs and PTs must have a minimum of 1 year experience with OASIS-C
Current state license in good standing required
American Heart Association CPR certified required
