is dedicated to helping elder and disabled adults live with dignity and independence. Our Structured Family Caregiver model has three main components:
24X7 at-home care from a paid caregiver (often a family member) living with the consumer; clinical oversight by an RN and Care Manager (typically a licensed social worker); and daily caregiver reporting in our web-based case record.
The Caregiver Homes service model is available to Medicaid beneficiaries in Massachusetts, Rhode Island, Ohio and Indiana (2012), and is demonstrating reduced hospitalizations and incidents (such as falls), with overall high integrity and excellent consumer satisfaction.
We continue to grow and have an opportunity in our Southeast region for a
Care Manager: P! er Diem, (English and Spanish or Portuguese fluency strongly p! referred).
The Care Manager will work collaboratively through the Southeast region (Fall River, Dartmouth, New Bedford, Middleborough, Carver, etc.) as part of a case management team to assess consumers, to establish and implement a Plan of Care, to conduct regularly scheduled homes visits, and to oversee consumer care. The team credentials caregivers and their homes, and provides ongoing support and training.
Qualifications:
Minimum of two years' experience in case management, care planning and assessment for elders/disabled adults in a health care, long-term care, social service or community setting.
Candidate must have a bachelor's degree and be licensed in social work or nursing, or an equivalent combination of education and experience in these or related fields.
Ability to speak, read and write fluently in both English and Spanish or Portuguese is
highly preferred
.
Skills/Knowledge Required:
Knowledge of case management systems, community service and delivery systems for aging population required.
Strong clinical and communication skills are essential with competent use of technology applications.
Regular car travel within assigned regional geography is required.
