Safety occurrence reporting system and investigations related to adverse events are managed in accordance with department and regulatory guidelines.
Receives, reviews, ! and codes Safety Occurrence Reports.
Manages investigations related to adverse events, including interviewing staff, reviewing medical records, conducting root cause analysis, documenting findings, and managing follow-up.
Reports adverse events to regulatory bodies (DPH, BRM, FDA, etc.) according to specific requirement.
Assists with coordinating DPH surveys
Refers cases for peer review.
Meets with patients and/or families to discuss adverse events
Refers appropriate cases to the Risk Management Foundation for monitoring, investigation or defense.
Counsels hospital employees as necessary with regard to legal proceedings in which they are professionally involved.
Patient Satisfaction Program is implemented and monitored to achieve stated goals.
Analyzes data obtained through Satisfaction Program. Identifies opportunities for improvement and works with management team to develop plans for improved performance.
Investigates serious custo! mer related complaints with members of the interdisciplinary t! eam.
Prepares written reports of complaints and actions taken
Facilitates follow-up process
Performance improvement and patient safety activities implemented to address quality concerns identified thorough hospital systems or by external reviewers to meet expected outcomes.
Participates in Performance Improvement & Patient Safety Teams as Leader/Facilitator or team member as assigned.
Performs clinical chart review of potential quality concerns identified internally through hospital systems or by external reviewers; i.e., Masspro, managed care providers, referring facilities.
Discusses findings of clinical chart reviews with appropriate managers and physicians.
Identifies and reports risk management concerns. Collaborates with clinical teams to design programs to prevent medical error, reduce loss, enhance quality and safety and assure regulatory and standards compliance..
Facilitates Proactive Risk Assessment/FMEA as assigned.
Facilitate! s data collection, analyzes aggregate data and develops reports to support hospital performance improvement and patient safety initiatives.
Maintains a close liaison with management team to assure coordination, standardization and continuity of performance improvement activities
Corporate compliance program is supported by performing audits and investigations, developing and shepherding action plans, and conducting related educational programs to meet organization and regulatory guidelines.
Researches regulatory requirements as needed to support departmental projects
Performs audits for a variety of risk areas
Investigates and responds to reports of compliance concerns form direct and anonymous sources
Accreditation surveys preparation and coordination is provided to ensure regulatory compliance.
Gathers, disseminates and documents information on patient care quality and safety to facilitate compliance with requirements of accrediting and regulat! ory agencies.
Performs regulatory compliance audits as assigned Assist management team in developing action plans to meet standards and regulatory compliance
Staff education is provided to build organizational capability to meet regulatory guidelines.
Provides advice, consultation and support to hospital and professional staff on performance improvement, compliance, accreditation, patient safety, patient satisfaction, risk, loss prevention and ethical issues.
Assists with presenting hospital wide educational programs related to above.
Assist with evaluation effectiveness of educational programs and improving presentations
Special projects and counsel are provided to department managers as needed to maintain compliance with organization and regulatory standards.
Potential quality or compliance concerns identified internally through hospital systems or by external reviewers (i.e., Masspro, managed care providers, referring facilities) are evaluated via clinical chart review anddata collection, with
Qualificati! ons and Experience

Education/Degree
Required
Bachelor’s Degree in Nursing or Health Care required.
Education and/or experience with performance improvement or risk management required.
Preferred

Experience
Required
Five years patient experience required.
Preferred
Three to five years related experience preferred.

Licensure
Required
Current Massachusetts’s license as Registered Nurse or other health care profession required
