Our local branch is currently seeking skilled

Registered Nurses (RN)

to work within our clients' homes providing direct patient care.

Currently, we have a pedi boy in Feeding Hills with a central line. His available hours are Tuesday - Friday 11a- 5p.

As ! an RN, you will be responsible for following a physician estab! lished plan of treatment under the direction of our Director of Clinical Services. Centrus believes that qualified nurses are advocates and health educators for patients, families and communities.

Responsibilities and Duties

Completes an initial assessment of patient and family to determine home care needs when applicable. Provides a complete physical assessment and history of current and previous illness (es)

Regularly re-evaluates patient nursing needs

Initiates the plan of care when directed and makes necessary revisions as patient status and needs change

Uses health assessment data to determine nursing diagnosis

Establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process

Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as pres! cribed by the physician

Provides health care instructions to the patient as appropriate per assessment and plan of care

Identifies discharge planning needs when applicable

Adheres to Centrus policy and procedure

Attends case conferences, team meetings, education in-services as required

Qualifications

Minimum Qualifications

Graduate of an accredited school of nursing

Currently li! censed as a Registered Nurse in the state

Complies with accept! ed professional standards and practice

One year prior professional nursing experience, home care experience preferred

Current CPR

PPD or chest x-ray if applicable

Successful completion of Centrus screening examinations with a score of 80% or greater

Participation and completion of Centrus Competency Program
