Tufts Health Plan provides a spectrum of health management programs for members with chronic and/or complex medical conditions. Reporting to the Care Mana! gement Team Manager, the Disease Manager Nurse will demonstrate strengths working independently as well as collaboratively within a team environment. An understanding of the Department business objectives and outcome measures is important to the accountability of the Disease Manager Nurse and the success of the Tufts Health Plan Health Management Program. The Disease Manager Nurse will provide telephonic intervention to support members with chronic illness including administer a comprehensive disease specific assessment to determine the member's specific opportunities for intervention. The goal of the Tufts Health Plan Health Management program is to provide education and health coaching to improve understanding and self management of the individuals' chronic disease/condition. Support to the members, both adult and/or pediatric, who participate in the program, will result in decreased unnecessary hospital admissions and decreased emergency room visits and improved disease ! self management for the individual member.

Qualificat! ions (Minimum education & experience level required): Education: Registered Nurse with current unrestricted state license required. Bachelor's in Nursing Required. National Certification in Disease or Case Management is highly desirable.Experience: Minimum of five years of clinical nursing experience in various settings with a focus on chronic health conditionsMinimum of 2 years experience required in disease management or chronic condition education specialist. For this position specific experience with heart failure and/or chronic obstructive pulmonary disease is required.Proficiency in a second language is highly desirableSkill requirements (include complexity of position and specify whether the job regularly requires independent judgment and discretion): Address multiple complex issues throughout the workday in a flexible professional manner.Identify emergency situations and follow the appropriate protocolPrioritize work to meet the needs of the member, the Department, ! and Tufts HP CorporateKnowledge and experience in effective disease management interventions and strategies across populationsDemonstrate strong clinical and critical thinking abilitiesExcel in positive relationship building.Effective listening skills, written and verbal communication abilitiesSkilled negotiatorPerform critical analysis, plan, organize, promote client family autonomyComputer literateInterpersonal skills (include personal interaction): Ability to perform job functions with autonomyAbility to collaborate with team members, medical providers, department management on job expectations and outcomesMust possess excellent interpersonal skills with an understanding of differences between the need to promote change while maintaining positive rapport with members, family members, and providers.Must develop and maintain positive communication to promote managed care philosophyMust have commitment to excellence in customer serviceConfidential data: All information (wri! tten, verbal, electronic, etc.) that an employee encounters while worki! ng at Tufts Health Plan is considered confidential.
