JOB DESCRIPTION: Care Manager (Nurse, RN) Always Here Home Care, Inc. is a non-medical home care agency located in the Back Bay of Boston. We are currentl! y seeking to add a qualified and experienced Care Manager (RN). The Manager would coordinate care between client representatives and our caregivers who provide non-medical care to the elderly and disabled with a focus on hospice, companionship, personal care and homemaker service.
POSITION: Care Manager (RN) will ensure the overall satisfaction of the client and client representatives with regard to the care being provided by Always Here Home Care, Inc. and the caregivers placed in the clients ’ homes. Care Manager (RN) is responsible for ensuring that services are being provided in a timely, safe, compassionate and caring manner to clients, in accordance with all Always Here policies and procedures. The Manager also interacts with the caregivers to ensure they are being treated fairly and that they have all the necessary education and supplies to safely and effectively provide care to the client.
Throughout the client and caregiver matching proce! ss, the Manager coordinates with case managers, social workers! , physical therapists, etc. to provide the best care possible. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Perform Client Assessment • Utilize assessment information as the basis for client-specific Care Plans; shares findings and planned actions with client representatives and office staff to promote a collaborative approach for meeting client ’s needs. • Coordinate with the client representatives, care managers, caregivers, and office staff as necessary to provide continuity of care for clients.
- Promotes education of caregivers on community events, senior supports and services, and proper caregiving techniques. • Coordinates and oversees the work of the Client/Caregiver Liaison and external caregivers. • Provides ongoing performance and competency evaluation including appropriate feedback to the Human Resources Manager • Quarterly Caregiver Reviews • Client Journals " Caregiver Emails • Schedule or fill emergency caregiving shifts if sched! uler is not available. • Afterhours and weekend phone coverage (full-time as back-up or short shifts as-need-be) KNOWLEDGE AND SKILLS: • Registered Nurse licensed in the Commonwealth of MA • BS in Nursing or appropriate degree in related field preferred • Minimum of 4 years ’ shared clinical and case management experience • Excellent Written and Verbal Communication Skills • Must be proficient with Microsoft Excel, Word, and Outlook • Possess cultural awareness and sensitivity, respectful of cultural differences • Strict adherence to all relevant legislation, policies and practices
