Qualification! s: RN with experience in clinical outpatient medicine to lead team of patients with opiate addiction/on suboxone. Familiarity with medical home model, , Spanish speaking and behavioral health and addictions experience a plus.
Position: Full Time, with some evening hours

Description: Position available to join a diverse Adult Medicine practice delivering high quality outpatient primary care in a community setting. The successful candidate would lead a medical home team with a focus on managing patients on suboxone and leading the suboxone program at Dimock. This position would work with other Adult Medical nurses, MA’s, front desk staff and ancillary staff to deliver direct patient care and organize population management . In addition, the nurse in this position would organize the systems around monitoring and supporting our suboxone clients and lead the efforts in ! quality improvements in this team.
