Job Details: The staff visiting nurse partners with the Case Manager to coordinate patient skilled nursing, preventative and rehab nursing care.
Counsel the patient and family to educate and participate in the treatment plan. Communicate with physician, Case Manager and other personnel of changes in the patient’s condition and needs.
RN with current MA license Visiting nurse home care experience preferred Clinical, teaching and customer service skills Organizational and problem solving skills Basic computers skills and ability to learn a new program
