Reports to: Medical DirectorGeneral Summary: Compass Medical is one of the largest groups of primary care providers with so! me 90 Family and Internal Medicine physicians practicing out of 7 office sites across southeastern Massachusetts. Affiliated now with Steward Health System and previously affiliated with Partners Health System, Compass has an outstanding reputation for high quality care outcomes and use of information technology. The Medical Director is seeking a Patient Centered Medical Home (PCMH) Facilitator to help all Compass Medical practice sites evolve into new PCMH models of care. The successful candidate will be a Registered or Advance Practice Nurse with an in-depth understanding of National Committee for Quality Assurance (NCQA) PCMH standards and applying these standards to primary care settings in transition from traditional to PCMH models of care.
He/she will have outstanding judgment about primary care practice operations and will be a resourceful problem solver when faced with practice operational challenges. He/she will have some understanding of managed care prin! ciples and how the PCMH can help physicians and care teams pro! vide optimal care to whole populations of patients within the primary care setting. Excellent interpersonal communication skills are essential as this position requires frequent face-to-face interaction with physicians, administrators, practice site managers, patients, and family members. The position requires the candidate to have a strong working knowledge of Microsoft Word, Excel, and PowerPoint for preparation of reports, data analyses, and presentations.Principal Duties: Develop a series of PowerPoint training materials on "PCMH Fundamentals", "PCMH Practice Readiness", and "Moving from Traditional Primary Care Practice to the Patient Centered Medical Home" and then conduct multiple training sessions for Compass Medical physicians, site managers, and staff within their practice sites.Develop and deploy a PCMH Readiness Assessment tool to benchmark where Compass Medical practice sites are in relation to NCQA PCMH standards.Lead efforts to analyze, gather, and submit dat! a to NCQA enabling Compass Medical to receive NCQA Heart/Stroke Recognition and NCQA Physician Practice Connections Recognition.Successfully transition all Compass Medical practice sites from traditional primary care practices to newly formed Patient Centered Medical Homes.Lead efforts to analyze, gather, and submit data to NCQA enabling Compass Medical to receive the highest level of NCQA Patient Centered Medical Home Recognition.Monitor progress and facilitate interventions needed after PCMH evolution has been completed.Minimum Requirements: Current Massachusetts licensure as Registered or Advance Practice Nurse.3 years minimum experience within an established PCMH especially one in which a leadership or facilitation role was played in transition from primary care setting to PCMH.Proficient use of Microsoft Word, Excel, PowerPoint, EMR, and web-based tools.Ability to travel frequently to Compass practice sites for numerous meetings and training sessions.__________________! ______________________ Please apply on-line.
