Qualifications
Job Summary
Ensure the delivery of high quality, safe and cost effective patient services through management of the weekend clinical and scheduling staff. Ensure a smooth transition between the weekend and weekday assignments through planning and communications procedures. Ensure field staff resources are deployed to maximize timely patient visits and continuity of caregivers being assigned to individual patients. Ensure regulatory and organization policies and standard are met.
Schedule (subject to change)
Friday 4 p.m. - 8 p.m.
Saturday and Sunday 7:30 a.m. - 5:30 p.m.
Monday 8 a.m. - 12 p.m.
Key Responsibilities
Manage clinical staff assignments to individual patients ensuring the highest level of care based on clinical competencies.
Support the practice of providing continuity of caregivers to patients.
Direct management responsibilities for the weekend staff which includes field cl! inicians, clinical coordinators and schedulers.
Provide clinical guidance and coaching for complex patient situations.
Staff assignments are managed to ensure productivity of clinical staff with minimal patient wait time.
Staff assignments will ensure efficiency in clinicians' drive time and mileage.
Capacity will be maximized by balancing staff assignments and reassignments to other geographic teams and areas, while meeting clinicians' productivity requirements.
Respond to incoming telephone inquiries from patients, families and staff.
Field supervisory visits and performance assessment of clinical staff occurs regularly.
Manage quality and fiscal outcomes, service utilization, targeted case mix and productivity levels.
Reports are prepared and information is analyzed to manage patient care and operational performance.
Provide back-up coverage for clinical coordinators and schedulers as needed.
Must be available to attend selected ma! nagement meetings during the week as requested.
Supervisory/Managem! ent Responsibilities Supervise approximately 15 clinical field staff (RN's, PT's, etc.), 3 Nursing Coordinators and 4 clerical schedulers - all nonexempt staff
Qualifications and Experience
Bachelor's degree in Nursing required or equivalent clinical experience.
Current license as Registered Nurse in Massachusetts
Minimum 5 years progressively responsible clinical experience
Minimum one year of home care
Previous supervisory experience required
In-depth understanding of home health Medicare Conditions of Participation, payment mechanisms for home health
Computer proficiency in Microsoft Office Suite and ability to learn new software.
Strong written, verbal and electronic communications skills
Exceptional customer service skills with special sensitivity to the patient experience
Strong analytical ability and prioritization skills to compile and analyze data to identify key trends.
Highly developed collaborative and relationship! -building skills to work with a diverse population.
Ability to critically assess a situation, make a decision and execute a solution.
Operate with integrity and possess management style that is ethical, honest and objective.
Local travel required.
