AFC Registered Nurse

The AFC RN is an experienced clinician who works with the Adult Foster Care team to assess client care needs and implement appropriate service plans for the enrolled clients. This RN makes home visits according to program guidelines.
RESPONSIBILITIES:
1. Adheres to AFC client visit standard and completes related documentation and any follow up required in a timely manner.
2. Completes assessments of potential clients and reassessments of enrolled clients per program guidelines.
3. Evaluates care needs and develops a! nd reviews on an on-going basis each clients individualized mu! ltidisciplinary care plan
4. Performs other duties as assigned and complies with all policies and procedures of the Agency and other regulators.
5. Develops and implements in-service programs on medical issues for staff.
6. Maintains collaborative relationships with agencies and institutions that serve BSHC clients.
7. Assists in program outreach.
8. Attends departmental and committee meetings.
9. Participates in activities that enhance self-competence and the Agency’s performance improvement program.
10. Review Physician Orders.
11. Monitor the health status of all clients.
12. Train, evaluate and supervise caregivers in conjunction with the Care Manager
QUALIFICATIONS:
1. Graduate of an accredited nursing school with a valid MA RN license. BSN preferred.
2. Must have two to three years of clinical experience, one of which must be in community setting. Experienced in home care nursing preferred.
3. Must have! sensitivity to issues affecting elders, and the mentally and physically impaired, plus a commitment to helping program participants maintain independent living with dignity.
4. Must be reliable and have the ability to work independently in a team.
5. Must be ready and available to work a regular full-time schedule..
6. Must be able to carry and lift a load up to 20lbs.
7. Must be computer literate.
