Care Manager

The Care Manager will work collaboratively throughout the Boston 1 area (Revere, Chelsea, Boston, Newton, Lexington, etc.) as part of a case management team to assess clients, establish and implement a plan of care, conduct regularly scheduled homes visits, and oversee client care. The team credentials caregivers and their homes, and provides ongoing support and training.
Qualifications:
- Minimum of two years’ experience in case management, care planning and assessment for elders/disabled adults in a health care, long-term care, social service or community setting.
- Candidate must have a bachelor’s degree and be licensed in social work or nursing, or an equivalent combination of education and experience in these or related fields.
- Ability to speak, read and write fluently in both Spanish and English is preferred .
- Knowledge of case management systems, community service and delivery systems for aging population required.
- Strong clinical and communication skills are essential with competent use of technology applications.
- Regular car travel within assigned regional geography is required.
