Clinical Social Worker: Behavioral Health
Duties:
Brockton Neighborhood Health Center is looking to fill an exciting new position for a clinical social worker who functions as a core member of a collaborative primary care team involving the patient, the patients’ primary care provider, psychiatry, nursing, and community health workers. Responsibilities include providing assessment, consultation, and brief interventions for psychological/psychiatric problems/disorders and diagnostic clarification utilizing the DSM4TR; evaluating crises situations and applying appropriate interventions and treatment planning; assisting the primary care team in developing care-management process such as disease management techniques and patient education to improve self-management of chronic diseases; provide and array of clinical interventions to assist patients in meeting treatment goals.
Requirements/Qualifications:
Experience in evidence based interventions (i.e. CBT, Motivational Interviewing, Solution Focused) required. Must have a MSW or masters in Counseling and be licensed or license eligible in the state of MA. Language and cultural diversity (Cape Verdean Creole, Portuguese, Haitian Creole, French, and/or Spanish) preferred.
