Description: Cambridge Health Alliance is employing CLIN. SOCIAL WORKER right now, this occupation will be placed in Massachusetts. Serve as a member of a multi-disciplinary staff team with nurse care managers (RNCM) and community health workers (CHW) to identify and intervene with high risk/high cost patients. These patients typically require support to utilize the healthcare system optimally and engage effectively in their individualized health plan. Identified patients will likely carry two or more co-morbid diagnoses which may include psychiatric illness, alcoholism and/or substance abuse.

Duties include:
1. Works in partnership with complex care management team to identify high risk/high utilization patients.

2. Screens patients for mental health/substance abuse problems and psychosocial factors impacting care.

3. Collaborates with payers, caregivers and staff beyond immediate CCM team to provide systemically informed care and health management for individual patients with complex needs.

4. Links and collaborates with staff at anchor service site, to foster a meaningful and collaborative approach to health management for identified patients.

5. Demonstrates a capacity to participate in a collaborative multi-disciplinary team process, with an ability to establish a meeting agenda and support the team through care planning for multiple patients with psychosocial, behavioral and medical needs.

6. Identifies and manages risk for specific populations ¿ homeless, substance using, elders, disabled, etc¿

7. Identifies and effectively utilizes community based services to support patient¿s needs (i.e.¿AA, SMART Recovery).

8. Collaboration will require participation in caregiver conferences, patient at-risk meetings, and other meetings as needed.

9. Engages family members to assist in assessment and care planning

10. Collaborates with mental health providers to support medical management in the context of mental health and substance abuse disorders.

11. Completes managed care documentation, statistics and other required paperwork thoroughly and on time.

12. Provides all necessary documentation in patient record in an effective and timely fashion.

13. Participates in activities of the Cambridge Health Alliance Department of Social Work.

14. Participates in teaching and training activities, including continuing education activities needed to maintain yearly licensing requirements.

15. May provide supervision for Community Health Workers and/or social work interns. May also provide clinical social work guidance for staff of other disciplines.

16. Will participate in PI activities, both in the service line/site and Department of Social Work.

17. Facilitates access to navigate federal, state and community entitlement programs, insurance benefits, and Department of Public Health and Department of Mental Health services. Supports patients/clients/CHWs with applications and assists with follow-up as appropriate.

18. Performs other duties as assigned by supervisor, including providing evening and weekend social work coverage when assigned.

19. Practices according to the ethical standards of CHA policies, JCAHO standards, and the NASW Code of Ethics.

20. Identifies the unique needs and implements appropriate interventions for the age-specific categories:

1. Pediatric

2. Adolescent

3. Adult (18-59)

4. Geriatric (60 +)

21. Performs other related duties as assigned.

Education/Training: Masters degree in Social Work or an equivalent degree in a related field.

Licensure: Current MA licensure as LICSW (Licensed Independent Clinical Social Worker) or eligibility for licensure. LCSW (Licensed Clinical Social Worker) is adequate when pursuing required supervisory hours in order to be eligible to register for LICSW exam.

Certifications: Same as above. CCM certification preferred.
