Alternatives Hospice is a licensed Medicare! and Medicaid
home health agency offering an array of services to help
our patients achieve their goals of care. We provide a
full range of services from in home skilled nursing
services, assistance with activities of daily living as
well as physical, occupational and speech rehabilitative
services by a team of highly-trained compassionate
professionals.
Our program is growing and we are looking for skilled
and compassionate caregivers to join our
interdisciplinary team. We are committed to patient
centered care. We understand how important it is for
patients to remain at home during their recovery.
TotalCare Home health of MA has an immediate opening for
an experienced Per Diem Home Care Registered Nurse (RN)
in our North Shore territory.

Essential Duties and Responsibilities:

• Provides comprehensive assessment and skilled

nursing care to patients in need of Home Care
ser! vices in accordance with plan of care.

• Accurately! documents care provided and maintains

patient records.

• Act as a Case Manager to coordinate plan of care;

communicates with physicians, insurance providers
and company staff.

• Supervises Home Health Aides and participates in the

training of Home Health Aides.

• Assist with company programs, shares in providing 24

hours, 7 days a week coverage to patients.

• Available as needed for assignment to meet the

changing needs of patient care 24 hours a day, 7
days a week.

• Advises and consults with the family and other

company personnel, as applicable.

• Works as a member of the interdisciplinary team.

Job Requirements:

• Must have an Associates Degree in Nursing,

Bachelor's Degree preferred

• Must have 2-4 years experience as a Home Care Nurse
• Must have a Valid MA Driver's License! & a reliable

source of transportation

• Strong follow through, interpersonal, oral & written

communication skills

• Ability and willingness to travel locally
