Qualifications
Job Summary
Referral requests are assessed for clinical appropriateness to determine acceptance for home health services and screened to ensure certified home health guidelines are met. Receive and document medical orders, accept for services, and fully process referrals. Review and reconcile physicians' orders and update medical data in the computerized system. Communicate pertinent information to involved parties according to Intake and agency guidelines. Primary contact with physicians for current and potential patients. Ensure that physician orders are received and communicated to appropriate staff. Maintain professional and collaborative external relations with referring physicians.
Qualifications and Experience
RN or LPN license to practice in Massachusetts required. RN license strongly preferred.
2 years medical-surgical experience in acute setting ! or 2 years experience in a community home health, discharge planning or managed care environment required
Computer literate with ability to learn new software applications
Strong written, verbal and electronic communications skills
Highly developed interpersonal skills with ability to interact with a diverse population
Strong organizational skills with aptitude for obtaining detailed information accurately
Understanding of insurances preferred
Must be available to work weekends and holidays
