The Program Manager of Quality role serves as primary change agent to plan for, implement and maintain NCQA Certified Patient Centered Medi! cal Home standards (PCMH) for HCPA. This position will develop and maintain excellent working relationships with HCPA patients, physicians and practice managers along the entire continuum of care (inpatient, outpatient, SNF/LTC, post acute). This role will act as primary resource to all HCPA practices to help troubleshoot and improve clinical workflows and gain efficiencies in support of PCMH guidelines and patient outcomes, and help practices improve their communication and coordination of all entities involved with the care of a patient to promote best possible outcomes and improve patient and provider satisfaction. Additionally, this position will direct the clinical efforts of HCPA Clinical Site Coordinators in support of PCMH and best practice clinical workflows. Registered Nurse with Bachelor’s Degree and current Massachusetts license strongly preferred. Candidates with a valid and unrestricted Licensed Practical Nurse license will be considered, pending experience.! Current CPR certification required. One to three years in a m! edical office or similar clinical/private practice setting, and private ambulatory outpatient experience preferred.
