Position Summary:
Provides oversight of medication self- administration and education to patients in home setting.
Coordinates medication administration activities with Care Team members and community resources.

Responsibilities:

  • Develops creative resources with patients and clinical team for medication adherence.
  • Evaluates the impact of the home environment on the health and safety of patients.
  • Establishes plan of care jointly with primary care nurse, patient, and family.
  • Collabora! tes with community resources to ensure medications are pre-pac! kaged.
  • Provides cueing and monitoring for medication administration.
  • Provides education to patients, family members and staff on cueing and monitoring.
  • Observes patients and charts and reports changes in patients' conditions, such as adverse reactions to medication or treatment, and takes any necessary action.
  • Monitors medication adherence, including side effects, tolerance and indications.
  • Performs medication reconciliation.
  • Observes self -administration of prescribed medications, and notes times and amounts on patients' charts.
  • Measures and records patients' vital signs as required.
  • Works as part of a health care team to assess patient needs, plan and modify care and implement interventions.
  • Utilizes community and agency resources as needed.
  • Collaborates with Nurse Practitioner on all cases to ensure delivery of quality patient care.
  • Assures accurate and timely documentation! of care per company standards.
  • Keeps abreast of all changes by attending team, full staff, mandatory and hoc meetings.
Requirements:
  • Minimum of 2 years clinical experience working with people with serious mental illness
  • Must hold current LPN License to practice in Massachusetts or be certified MAP provider
  • Current CPR Certification
  • Bilingual (Spanish) preferred
  • Ability to make visits in a home-based setting and access to reliable transportation
  • Sufficient visual/hearing ability, with or without reasonable accommodation, to comprehend written/verbal communication and perform physical exams and assessments of patients
  • Ability to take direction from a number of providers on the team
