The Cooperative for Human Services, Inc. is a non-profit organization that provides residential services and supports to adults with cognitive and intellectual disabilities throughout greater Boston. We are seeking a full-time LPN to provide medical case management (attending medical appointments, liaison with medical providers, coordination with the CHS team, etc.), training and program support within our residential services. The LPN, as supervised by our RN, will work cooperatively with program ma! nagement to identify healthcare needs and work to actively mee! t those needs. The position is full-time with a Monday - Friday, primarily daytime work schedule. Part-time or per diem work schedules will also be considered. Eligibility for benefits (health, dental, paid time off, etc) is based upon a minimum of 20 hours per week.
