Outpatient Care Case Manager
Under the direction of the Program Manager! , Care Management, the Outpatient Care/Case Manager is responsible for a wide variety of duties related to care and case management. Major responsibilities/accountabilities to include accurately and efficiently identifying patients appropriate for care management, and initiating and performing care management processes in a quality focused, cost-effective manner across the continuum of care. The incumbent must effectively communicate with patients, physicians and their staff on a regular basis, as well as be attentive to detail to maintain accurate and timely data exchanges between all entities involved in the patients’ care. Appropriate documentation in patient medical records and/or care management application is required and is vital.
The Outpatient Care/Case Manager plays a key role in implementing Baycare activities to assist practices in achieving NCQA PCMHTM Recognition. Strong knowledge of practice work flow is critical.
Qualifications
R! N licensed in the Commonwealth of Massachusetts is required al! ong with a minimum of three to five years of clinical related job experience. Bachelors Degree preferred.
Outpatient nursing and / or care management experience strongly preferred. Experience with educating patients and patient goal setting is essential. Healthcare experience with MCOs, IDSs, PHOs or POs preferred.
Qualified candidates must have excellent written and verbal communication and interpersonal skills
